Benefits
HealthCare Accessories contributes toward the cost of employee benefits, which is based on full-time or part-time employment status. An employee is considered full-time, for benefit purposes, if scheduled to work at least 64 hours per pay period. An employee is considered part-time, for benefit purposes, if scheduled to work for 32-63 hours per pay period.
Health Insurance

HealthCare Accessories offers North Dakota Blue Cross/Blue Shield health care coverage to full-time and part-time employees. Enrollment in a health plan may begin upon employment at HealthCare Accessories. HealthCare Accessories contributes a designated amount toward the cost of coverage premium. For part-time employees, the coverage premium is prorated according to scheduled hours worked.

Dental Insurance

Dental insurance coverage for full-time employees is available through the Dental Service Corporation of North Dakota, administered by North Dakota Blue Shield. HealthCare Accessories contributes a designated amount toward the cost of dental coverage.

Flexible Benefits Program

Enrollment into the Flexible Benefits Program is available after three months of employment with HealthCare Accessories. January 1 of each year begins a new Flexible Benefits Plan Year. The Flexible Benefits Program entails two parts.

  • Premium Conversion: This allows the employee to use pretax dollars to pay for their share of insurance premiums.
  • Flexible Spending Accounts: This allows the employee to set aside pretax income to pay for childcare or medical services. Employees have the option of designating a specific portion of their salary into one or both kinds of flexible spending accounts:
    • Dependent care spending account (limited to $5,000/year)
    • Medical spending account (limited to $2,500/year)
Life Insurance

HealthCare Accessories provides full-time employees term life insurance coverage of $25,000.

Disability Income Protection
  • Short Term Disability: HealthCare Accessories provides a Short Term Disability Plan, which accrues hours into the employee's account from their date of hire. Short Term Disability is an accrued benefit for full-time and part-time employees.
  • Long Term Disability: HealthCare Accessories provides full-time employees long term disability insurance coverage. Enrollment eligibility is after six months of employment.
Paid Time Off (PTO)

Health Care Accessories provides a Paid-Time-Off (PTO) program which gives the employee the flexibility in using time off and at the same time the responsibility of managing their PTO.

PTO hours are earned based on actual hours of service for employees working more than 32 hours per pay period. The accrual rate at which hours are earned is affected by the employee's hours of service.

Pension Plan

HealthCare Accessories helps their employees plan for the future by providing a retirement plan. Employees are enrolled in the pension plan after completing one year of service in which 832 or more hours are worked and are 21 years of age.

HealthCare Accessories contributes into the retirement plan an amount equal to six percent of the employee's total earnings each year the employee works 832 hours or more.

401(K) Plan

To assist employees with their financial planning for retirement, HealthCare Accessories offers employees the option to participate in a 401(K) Plan. Employees must be 21 years of age and have completed one month of employment to be eligible to participate. Enrollment is done online at http://retirementplan.wellsfargo.com. The amount of the employee's contribution will not be subject to state or federal income tax, but will be subject to Social Security tax. All contributions are 100% vested immediately.

Tuition Assistance Program

HealthCare Accessories offers employees the opportunity to advance their education through the Tuition Assistance Program. Reimbursement of tuition for academic credit is available if the course is directly related to your job, leading to a degree in a health care related field, or is health care related.

Full-time employees can receive up to $1,000 and part-time employees up to $500 per academic year. Employees are eligible for tuition reimbursement if scheduled for at least 32 hours per pay period and have completed twelve full months of employment. The employee must continue employment for one year following the completion of coursework.

Adoption Assistance Program

HealthCare Accessories offers an Adoption Assistance Program that provides reimbursement of qualified adoption expenses to employees who are adopting a child. Employees that have been employed full-time for six months or more are eligible to receive up to $2,000 per child, per family, for eligible adoptions.
