Your Financial Responsibility to MeritCare HealthCare Accessories

Please bring your Insurance Identification Card with you on each visit to our store or have the information available at the time of your phone order. The services you receive from us require that you have Durable Medical Equipment / Prosthetic Orthotic (DME/PO) coverage on your insurance plan if you want charges filed to insurance.

If you have Medicare you must be enrolled in Part B coverage. If you are unsure about this you can check your Medicare card and the effective date will be shown on it.

Most insurance plans require that you pay a deductible and coinsurance each year. A "covered service" will be applied to your annual deductible and coinsurance amounts. We request you pay for the services that you receive. This will be done at the time of your visit. Doing this will help ensure that you have met your financial responsibility with both your insurance company and MeritCare HealthCare Accessories.

Insurance companies may reduce benefits or not pay for services that they determine to be experimental, investigative, recreational use (such as sports), not medically necessary, or not provided within their provider network. Insurance policies may have product limits or plan exclusions. This does not mean that you do not need the equipment or services.

Medicare and insurance companies mandates that the "Date of Service" is the date you receive the product or service. This may not be the date of the order or casting.

If you have questions about your insurance plan you should contact the subscriber service number on the back of your insurance card.
